Cancellation Policy

All services are prepaid at the time of booking or employer authorization. Payment secures appointment time, collector availability, and/or a designated walk-in testing window. By submitting payment, clients acknowledge and agree to the terms outlined in this policy.

Scheduled appointments may be canceled with at least 24 hours’ notice and are eligible for a refund minus a $5 administrative fee. Cancellations made less than 24 hours before the appointment time are non-refundable. Failure to appear for a scheduled appointment without notice is considered a no-show, and prepaid fees are forfeited. Rescheduling after a no-show may be permitted at the provider’s discretion and may require new payment.

For employer-paid or prepaid walk-in services, testing must be completed within the designated testing window provided at the time of payment. If the individual does not present during the approved window, the service is considered unused. Unused prepaid walk-in services are non-refundable; however, payment may be applied as a credit toward a future test or replacement employee at provider discretion. Employer-authorized testing is non-refundable once payment has been processed and testing availability has been provided.

Fees for specialty kit collections, including blood draws and specimen processing, cover professional specimen collection, proper labeling and documentation, packaging in accordance with laboratory requirements, and drop-off to the designated shipping carrier. 

Collection fees are separate from laboratory testing fees and shipping carrier services.

Once a specimen has been properly collected, packaged per laboratory instructions, and transferred to the shipping carrier, Clear Code Testing is not responsible for carrier delays, missed delivery guarantees, weather disruptions, or shipping errors. Carrier performance and delivery timelines are outside the provider’s control. If a laboratory rejects a specimen due to shipping delays or carrier error, the original collection fee remains non-refundable as the service was properly performed. In such cases, a discounted recollection fee may be offered at provider discretion.

If a service cannot be performed due to medical, laboratory, or eligibility restrictions, a full refund will be issued. Approved refunds are processed back to the original payment method within 7–10 business days.

Clear Code Testing reserves the right to modify testing windows and availability as necessary. Submission of payment confirms acceptance of these terms.
